What are Care Management Services?

When standard outpatient care is simply not enough, we have the ability to meet the needs of our clients by providing a concierge approach to care that is individually tailored to you or your loved one. Research shows that practices which emphasize treatment within the community of origin have far better outcomes, and less trauma compared to the traditional model of psychiatric hospitalization, residential treatment and therapeutic boarding schools.

Our “whatever it takes” model of treatment

Upon referral to our treatment team, a member will schedule an initial screening assessment to determine a plan of care that may be right for you. After this initial assessment, we will produce a clear and transparent treatment plan with our recommendations, costs associated, and work with your insurance company to determine eligibility for out of network reimbursement (due to constraints of the insurance world, we are unable to utilize this as an in-network service).

The typical approach to care is assigning multiple members of our care team to meet the individual and complex needs of you or your loved ones. This team will be led by one of clinical leadership staff and could consist of psychiatry, family therapy, individual therapy, bio/neurofeedback and applied behavioral analysis. Patients should expect a combination of in-office, in-home, or school and community encounters with our staff, to ensure that interventions are applied globally.
