Famotidine (Pepcid): A Potential Treatment for Post-COVID-19 Neuropsychiatric Symptoms

Famotidine, a selective histamine H2 receptor antagonist traditionally used to treat acid reflux, is gaining attention for its potential neuroprotective benefits. A recent study titled "Effect of famotidine on cognitive and behavioral dysfunctions induced in post-COVID-19 infection: A randomized, double-blind, and placebo-controlled study" highlights how this medication may improve cognitive and emotional well-being in patients recovering from COVID-19.

What Is Famotidine and How Does It Work?

Famotidine works by blocking histamine H2 receptors in the stomach lining, reducing acid production and relieving conditions like heartburn and ulcers. However, its effects extend beyond the gastrointestinal system. Famotidine can cross the blood-brain barrier, allowing it to influence brain function.

In the context of neuropsychiatric symptoms, famotidine’s proposed mechanisms include:

  • Anti-inflammatory Effects: By reducing inflammatory markers like IL-6 and TNF-alpha, famotidine may help mitigate neuroinflammation linked to COVID-19-related brain dysfunction.

  • Immune Modulation: It may regulate immune pathways such as Toll-like receptor 3 (TLR3) signaling, which contributes to cytokine storms during severe infections.

  • Neuroprotection: Famotidine inhibits glycogen synthase kinase 3 beta (GSK-3β), a pathway implicated in cognitive impairment and neurodegeneration.

These mechanisms suggest that famotidine could play a role in reducing inflammation, protecting neural tissue, and improving cognitive function in post-COVID-19 patients.


How Famotidine Improves Cognitive and Emotional Symptoms After COVID-19

A recent double-blind study evaluated 50 patients with confirmed cognitive impairment following COVID-19 infection. Participants received either 40 mg of famotidine twice daily or a placebo for 12 weeks. The results were promising:

  • Cognitive Function: Patients treated with famotidine showed significant improvements in Mini-Mental State Examination (MMSE) and Montreal Cognitive Assessment (MoCA) scores at six and twelve weeks compared to the placebo group. MMSE scores increased by an average of 4.96 points over 12 weeks in the famotidine group versus 2.68 points in the placebo group. MoCA scores improved by 5.76 points compared to 2.92 points in the placebo group over the same period.

  • Depression Symptoms: On the Hamilton Depression Rating Scale (HAM-D), famotidine reduced scores by an average of 2.16 points over 12 weeks compared to a reduction of 1.24 points with placebo treatment.

  • Anxiety Symptoms: The Hamilton Anxiety Rating Scale (HAM-A) showed modest reductions in anxiety symptoms with famotidine treatment, with a decrease of 0.8 points over 12 weeks compared to 0.2 points in the placebo group.

These findings suggest that famotidine may help alleviate both cognitive deficits and mood disorders in patients recovering from COVID-19.


Safety Profile of Famotidine

Famotidine was well-tolerated during the study, with mild side effects such as nausea, dizziness, fatigue, headache, and diarrhea reported at similar rates between treatment and placebo groups. Importantly, no serious adverse events were observed.
